Physical Examination

Physical Examination taken after April 1st and thereafter is valid for the following two school years: physical examinations taken before April 1st are valid only for the remainder of the school year and the following school year. In the year when an examination is not required, an Alternate Year Permit Card must be signed by a parent or guardian and be on file.

Once you have your physical completed by a doctor, you can turn it into the Athletic office or you can scan it and submit the completed Physical Forms online.
